Medical marijuana certification is always self-pay. But pain management and opioid management are most likely covered by insurance.

Medical marijuana certification, follow-ups, and dispensary purchases are not covered by insurance under Alabama law.

All marijuana certification visits are cash-pay. Payment of $150 is collected when you schedule. Pain management visits are billed to insurance separately.

Refund Policy: If you complete the Do I Qualify screening and pay $150 at scheduling, but the physician determines you do not qualify after your evaluation, $100 will be refunded. $50 covers the physician’s consultation time. We encourage you to answer the screening questions honestly and bring medical records from other doctors so we can give you the most accurate assessment upfront.

Epilepsy / Seizures

2+ anticonvulsant failures.

Crohn’s Disease

GI-confirmed. Failed 5-ASA, steroids, or biologics.

Multiple Sclerosis

MS with spasticity. Failed muscle relaxants.

Parkinson’s Disease

Neurologist-confirmed. Failed motor symptom meds.

Persistent Nausea

Not pregnancy-related. Failed antiemetics.

HIV/AIDS Symptoms

Nausea or weight loss. Failed appetite stimulants.

Sickle Cell Anemia

Confirmed hemoglobin electrophoresis.

Spasticity — ALS

Motor neuron disease. Failed baclofen/tizanidine.

Spinal Cord Injury

Documented SCI with pain/spasticity.

Autism Spectrum Disorder

DSM-5 diagnosis. Failed behavioral/pharma interventions.

Terminal Illness

Prognosis < 2 years. No THC cap.

Don’t have a documented diagnosis? If you have chronic pain or another condition but no formal diagnosis, we can start the evaluation process at our pain management clinic — most likely most likely covered by your insurance. Once diagnosed, you can add marijuana certification as a self-pay service.

FAQ
Can I do this by telehealth?
No. Alabama law requires in-person evaluation. But we make it fast — complete your intake online beforehand so your visit is quick and focused.
Do you sell marijuana products at your office?
No. We certify you for your medical marijuana card. You then take your card to a licensed dispensary to purchase products. Our offices are near GP6 Wellness (Birmingham) and Callie’s Apothecary (Bessemer) for your convenience.
I’m already a pain patient at APP. Do I need a separate visit?
No. We can add marijuana certification to your regular pain management visit. Pain bills to insurance, marijuana certification is a $150 self-pay add-on. Same visit, same physician — no extra trip.
Can I take opioids and medical marijuana together?
In most cases, yes — with physician oversight. We specialize in managing both together, and can help you safely reduce opioid use if marijuana provides adequate relief.
What if I have pain but no diagnosis?
We can evaluate you at our pain management clinic first — most likely covered by insurance. If you have a qualifying condition, we can add marijuana certification as a self-pay add-on at the same visit.
What forms of marijuana are allowed?
Tablets, capsules, tinctures, gel cubes, topicals, transdermal patches, nebulizers, and inhalers. Smoking, vaping, and edibles are prohibited in Alabama.
Will my employer know?
Your status is HIPAA-protected. However, Alabama does NOT protect patients from employer drug testing. Your employer can still test for THC.
What about insurance coverage?
Marijuana certification and dispensary products are always self-pay. But pain management, opioid management, and diagnostic services bill to your insurance normally.
How much THC can I take?
50mg/day for the first 90 days. Your physician can increase to 75mg after a follow-up. Terminal illness patients have no cap.
